President Joe Biden was riding a leisurely bike in his home state of Delaware and when he came to a stop to greet supporters, he fell awkwardly to his right. As an orthopedic surgeon, I know this is a dangerous fall where the elderly can easily break their hips.

Fortunately, it did not matter as President Biden suggested, “I’m fine.”

This kind of spill can happen to anyone. Due to the inertia, it is definitely easier to fall at low speeds. Think back to when you taught or taught your children how to ride a bike. It is difficult to maintain balance at very low speeds. Biden puts his left foot down for balance as he stops. When he shifts his weight to the right, his toe is trapped in a cage attached to the pedal and he cannot lower his right foot at the right time. Thus, he falls on his right side / hip. The video did not show how he landed. Perhaps he was able to react quickly enough to prevent his fall and hip fractures.

This may come as a surprise, but the mortality rate in people over the age of 50 in the first year after a hip fracture is about 1 in 3, even with surgical stabilization and the latest medical advances. In fact with increased age and co-morbidities, that rate becomes even higher. Most of us have direct experience with our grandparents or parents.

The bottom line is that Biden is lucky to have a hip fracture, which is a very serious injury.
